Checklist for RISE Graduate Scholar Program:

A completed application packet must be received in the RISE office for consideration by July 1st (Fall) or November 1st (Spring) and must include the following:

  •  RISE Graduate Scholar Program
  • RISE Scholarship MS application form
  • (2) Faculty Letters of Reference
  • Personal Statement
  • Proposed Plan of Research (if applicable)
  • Official copy of undergraduate transcript in STEM major

Checklist for RISE Undergraduate Scholar Application:

A completed application packet must be received in the RISE office for consideration by March 30th and must include the following:

  • Completed, signed application (All questions must be answered).
  • Two completed rating forms. Evaluators must sign completed rating forms and place them in envelopes. Envelopes should be sealed and signed by the evaluator on the back flap. Evaluators may send recommendations directly to the RISE Office to expedite action.
  • Up-to-date official transcripts from all high schools attended. The transcripts must be received in a sealed envelope. School office may send transcripts directly to RISE Office.

Checklist for Pre-Matriculation application packages:

  • Completed, signed application (all questions must be answered).

Checklist for Sophomore Immersion Program in Research and Academics (SIPRA) application package:

  • Completed, signed application (all questions must be answered).
  • Two completed rating forms. Evaluators must sign completed rating forms and place them in envelopes. Envelopes should be sealed and signed by the evaluator on the back flap. Evaluators may send recommendations directly to the RISE Office to expedite action.
  • The transcripts must be received in a sealed envelope, unless student has provided mid-term and semester grade reports of the RISE office.
